Additional Guide Documentation

In addition to waivers and Guide Agreements, there are two more pieces of documentation we need to have on file for Guides.

Other items we need to have on file for Guides include:

• A copy of at least one current leadership-level certification card.
• Copy of your current Certificate of Insurance.

Note that the latter is your professional liability insurance (what you get through your training agency), and not your DAN or other recompression insurance. You also need to list the following parties as Additional Insureds on your policy:

• Commercial Diving Academy of Jacksonville, Inc.
• Hal and Jan Watts.

You can mail or fax this documentation when it is due, or you can just bring it with you on your next visit.
